兒童時計演出時的肖像權同意書

請審閱下列肖像權同意書後在報名。
(另外，關於肖像權等本身，請監護人直接與所屬經紀公司做確認)

我模特兒合格者本人，同意貴公司或貴公司所指名之人到指定的場所所拍攝的照片，以及動畫，關於我的肖像以及我所記載的自傳(以下稱之為「本肖像等」。)都可以無償使用。 

模特兒合格者本人為未成年者時，監護人同意下列所定條件。

【條　件】
  1. 使用媒體:網路･放送･廣告･印刷物･販售促進物･商品(寫真集,雜誌,書籍,DVD,成品等)等等
  2. 使用地區:無限制
  3. 有効期限:今天開始3年。
  4. 網路上的使用中止:從我提出要求3週以內。貴公司能刪除可能範圍內做刪除，已開始流通之媒體不在此限。/li>
  5. 提供樣本･媒體的提供:無
  6. 禁止侵害演出:上述第3項之有効期限內，如有使用於類似徵選比賽時，我肖像的使用需事先以書面方式得到貴公司的承諾。
  7. 兒童時計演出之經歷的使用:所拍攝的照片使用於貴公司以外的作品、商品、服務、包裝及廣告宣傳時，「含兒童時計(相關活動)」之照片，均須以書面事先得到貴公司的許諾方可使用。
  8. 其他:對於本肖像等的選擇、使用方式、編輯創作等無異議。
  9. 合意仲裁處:大阪地方法院

The agreement of right of publicity regarding to modelling at Kids-Tokei


(This applies to the only models who appears on Kids-Tokei)


The consent form of right of publicity for modeling at Kids-Tokei

Please apply for the model after reading ｔhe consent form of right of publicity.
(I, or the model himself, agree with the usage of photograph which are taken by the person whom Kids-Tokei has appointed to at the locations where Kids-Tokei has designated, the usage of my portrait in the images and also the usage of my profile which I have discribed (following, these will be called 'this portrait and others') gratuitously by the other parties which Kids-Tokei has appointed to.)

Also, I agree with the conditions below as a parental gardian of the model in case the model himself is infancy.

【Conditions】
  1. Media: Internet, broadcast, advertisement, printed materials, promotional materils, puroducts(e.g. photo album, magazines, books, DVDs, and goods)
  2. Area of usage：No limitation
  3. Effective Period：Three years from today
  4. Cancellation of the usage on the web: .Cancellation of the usage on the web should be done within three weeks from the day I claimed. In this regard, however, it will be only the range which Kids-Tokei is able to delete.
  5. Sample offer/Providing of media: unavailable
  6. Prohibition of modeling for competitors: I will obtain prior consent from Kids-Tokei in case I agree with the usage of my portrait for the business enterprises which are similar to Kids-Tokei or which competes with Kids-Tokei during the period which is described above in paragraph 3.
  7. Display of the modeling career of Kids-Tokei: I will obtain prior consent from Kids-Tokei in case I state the history of modeling career of 'Kids-Tokei (including the contents relating to Kids-Tokei)' on the package and on the advertising goods when I do modeling for the other productions which are not of Kids-Tokei and also when I agree with the usage of my portrait for their products or sevice.
  8. Other: I will not make an objection against the choice of the portrait, type of usage and optical creation.
  9. The agreed exclusive jurisdiction：Osaka District Court
